Any Form-based monitor can be triggered either by

  1. Embedding the Form into Slack

  2. Navigating to the Form URL and submitting there

Form URL

Find the Form's URL by navigating to the Monitors page > Selecting the Monitor > "Submit Form" button top-right.

This will open the Form URL for your users to fill out (see below)

From Slack

There are two ways to trigger forms from slack, you can either select it from the Avenue Slack Bot's selection forms, or create a workflow.

Avenue Slack Bot:

This is the easiest method as there is no setup, the only downside is you will see all of the available forms, they will not be specific to the channel you are calling them from.

  1. Type /avenue into the input box and press enter.

  2. Select the forms you would like to submit.

Creating a Workflow

This takes a little setup but allows you to create workflows for your team that are associated with specific Slack channels.

  1. Head to your Slack Workflow builder, choose legacy workflows then Create Legacy.

  2. Chose a Workflow Name.

  3. Select Shortcut.

  4. Then add the channel where you want to start the workflow and add a Short Name(label).

  5. Choose Add Step and search for Avenue, you will want to select Trigger an Avenue Alert.

  6. Add your final configurations and choose the form you wish to use in the workflow.

  7. Then click Publish! You are all set and can trigger the workflow from your channel by typing a forward slash: /Workflow-Name.
